Summary19374: Database error when adding more than one subquestion in multiple choice type.
Description

When a multiple choice question is created and more than one subquestion is added, a database error occurs.

Steps To Reproduce

Steps to reproduce

  1. Create a test survey
  2. Create a test question that is multiple choice
  3. Add more than three subquestions or answers to the question
  4. Click on save

Expected result

Survey and question saved correctly

Actual result

A database error occurs when inserting into a table
