Description

In QueXML PDF export the survey name is not displayed. It starts with the Welcome text.

Fill out survey, press button "QueXML PDF Export", look at the generated PDF

Expected result

in the generated PDF I expect as the first information - as the headline - the survey name.

Actual result

No survey name, PDF starts with welcome text.

For us to be able to investigate this issue please attach a 2 question sample survey demonstrating the issue, either as .lss file (survey structure) or (even better) as .lsa file (survey archive).
