Description

Hello,

When a form submitter reaches the survey group, they are prompted with several upload questions, only the first button clicked shows the "upload" button in the pop-up, in other questions it does not work.

Create a survey group with multiple file upload questions

Expected result

When the first file upload button is pressed, it displays the proper prompt to upload. When the second file upload button is pressed, it displays the proper prompt to upload.

Actual result

but other file upload buttons show a popup with just an upload title but no area to upload a file.
